Key Issues in Choosing a Quality Measurement Strategy

In her discussion of quality measurement at the September 1998 SCHIP Workshop, presenter Lisa Simpson reviewed key issues in choosing a quality measurement strategy, which are summarized below.

There are three basic dimensions to consider in choosing a quality measurement strategy:

  • Accountability. Who is responsible for quality?— the health plans? the providers?
  Provider Health Plan State/Community
Structure · % weekend hrs,
· days wait for WCC
· # pediatricians
· health plan stability
· % plans accredited
· educational campaigns
Process · % immunized
· treated w/respect
· % immunized
· asthma care
· lead screening
· % injury counseling
· % immunized
· EPSDT rates
· HIV education
· care for CSHN
· lead screening
Outcome   · preventable hospitalizations · school absenteeism
· injury rate
· % LBW
  • Time frame. Short- versus long-term. Will quality measurements be used for year-to-year monitoring or to plan longer term improvements?
  • Method. Standardization versus innovation. Although standardization facilitates comparison across States, differences between States and their SCHIP populations may necessitate innovative quality measures.
